The Johns Hopkins Women Infant Children and Youth Partnership for Excellence in HIV Care in Central Maryland (JH-WICY) is a regional program focused on assuring the best quality of life and health outcomes for women, infants, children and youth (WICY) living with HIV by improving access to treatment and supportive services and aiding the development of best practices. JH-WICY is seeking a Regional Outreach Coordinator (ROC) to assist in the pursuit of this mission.
The ROC will work under the supervision of the JH-WICY Program Manager. The incumbent will work directly with Ryan White eligible clients in the Baltimore-Towson EMA to engage and retain them in medical care and non-medical support services. The ROC will also provide limited support services to newly diagnosed HIV-positive clients and clients who have difficulty engaging in medical care. They will be responsible for building relationships with local health departments (HDs), partnering clinic staff, medical providers, support service organizations, and local stakeholders in Baltimore City and the six surrounding counties. More specifically, the job responsibilities of the ROC include:
Specific Duties & Responsibilities:
- Engage RW eligible client referrals, particularly youth, and engage them in HIV primary care and non-medical support services to increase retention.
- Assess clients' needs and make referrals to appropriate services, including medical care, social services, community, legal, financial, and other needed non-medical services and facilitate those appointments.
- Identify a clinical site that meets the client's needs to ensure coordinated linkage and engagement.
- Assist clients referred for care with setting up the first few appointments and also provide a resource for the patient to follow up on referrals and services within the health care system.
- Provide limited support services including, but not limited to, scheduling transportation, determining eligibility for emergency financial assistance, and providing incentives to engage in care.
- Maintain regular contact with the client caseload and develop individual care plans as needed.
- Assess for barriers that would prevent a client from attending the clinic for the first time (e.g., transportation, navigation of services) and work with the client to address those barriers.
- Document all contacts with or on behalf of the client.
- Maintain professional/personal boundaries with patients, keep the Program Manager apprised of all possible conflict situations, and maintain strict confidentiality of all patient information.
- Participate in workshops, seminars, and assigned training programs to facilitate ongoing professional development.
- Identify regional medical providers and support service organizations providing care to HIV-positive and at-risk youth.
- Create and maintain a log of youth-friendly providers and regional support services.
- Work with existing partners and medical case managers to expand our reach to the WICY population and facilitate linkage and retention in care, particularly during the early period of diagnosis and engagement in care.
- Provide technical assistance to partner sites to enhance engagement with the WICY population.
Minimum Qualifications (Mandatory):
- High School Diploma with a minimum of 4 years of experience working with HIV positive or at-risk population.
- Additional education may substitute for required experience, to the extent permitted by the JHU equivalency formula.*
- Personal means of transportation to execute job duties.
* JHU Equivalency Formula: 30 undergraduate degree credits (semester hours) or 18 graduate degree credits may substitute for one year of experience. For jobs where equivalency is permitted, up to two years of non-related college course work may be applied towards the total minimum education/experience required for the respective job.
Preferred Qualifications:
- Bachelor's Degree is preferable.
- Formal training in counseling is preferable.
- Experience working with people with unstable housing, substance use issues, dual diagnoses, and HIV/AIDS.
- Excellent writing and interpersonal communication skills, particularly for communicating with a wide variety of individuals from patients to administrative personnel, experiences, clinicians, and physicians.
- Ability to adhere to policy and procedures and maintain clear and professional boundaries.
- Attention to detail and ability to chart and enter data accurately.

The pre-employment physical for positions in clinical areas, laboratories, working with research subjects, or involving community contact requires documentation of immune status against Rubella (German measles), Rubeola (Measles), Mumps, Varicella (chickenpox), Hepatitis B and documentation of having received the Tdap (Tetanus, diphtheria, pertussis) vaccination. This may include documentation of having two (2) MMR vaccines; two (2) Varicella vaccines; or antibody status to these diseases from laboratory testing.
