The Baltimore City Health Department's (BCHD) mobile clinical services program Healthcare on The Spot (The Spot) prioritizes delivering specialized health care services to people who use drugs (PWUD) in Baltimore City. We are seeking a Case Manager who will be an integral part of a multidisciplinary team, working with other clinicians, non-clinical case managers, and community health workers to engage and manage clients living with HIV, hepatitis C (HCV), Opioid Use Disorder, mental health disorders, and acute, or chronic wounds. This position will spend most of their time in/on the mobile clinical services van, which parks at select sites in communities with high rates of opioid-related overdose, five days per week, as well as engaging with patients telephonically via our buprenorphine phone outreach program. This position requires someone who is self-motivated, highly organized, willing to work in mobile and community settings, flexible in learning new service delivery models and thrives in team environments.

Specific Duties & Responsibilities

• Provide program information to prospective patients regarding the BCHD HIV, hepatitis C (HCV), buprenorphine, mental health care, and wound treatment programs at the BCHD mobile clinic.
• Administer initial comprehensive psychosocial intake for new patients and document in clinic EMR in a timely manner. This will include administering screenings for mental health disorders.
• Navigate patients through clinic enrollment process for HIV, HCV, buprenorphine and mental health programs. This will include face-to-face visits and phone sessions, as needed.
• Develop and maintain a resource guide and referral list of primary care providers, behavioral health and addiction specialists in Baltimore City.
• Enroll clients in coordinated access and provide housing navigation support to interested clients.
• Navigate patients through establishing care with primary care physicians (PCPs), behavioral health specialists or addiction services as needed.
• Advocates and coordinates for clients with insurance companies, outpatient pharmacies and suppliers to ensure client needs are met. This includes in person, or virtual accompaniment, at appointments.
• Follows up with patients regarding their plan of care, or recent transition to a new care provider.
• Assists medical directors with program evaluation and quality improvement initiatives.
• Direct weekly group sessions for patients enrolled in buprenorphine program.
• Serve as driver for the mobile clinic (only standard driver's license required) and maintain general maintenance for the van.

• Alternate sitting and standing through the workday.
• Ability to operate a motor vehicle consistent with the requirements of the state of Maryland.

• Bachelor's Degree in social work, psychology or related field.
• One year of related human service experience, preferably in case management.
• Valid MD driver's license required.

• Active Maryland state social worker license (LBSW, LMSW, or LCSW).
  

• Proficiency in spoken and written Spanish is preferred.
  

• Experience working with people who use drugs or dealing with addiction is preferred.
  

• Community, case management, and/or outreach experience preferred.
