The Diabetes Coordinator oversees and facilitates the processes, policies and management provided by team members in the inpatient setting through education, coordination, timely assessment, and analysis of metrics.
Responsibilities
A day in the life of a Diabetes Coordinator with Hackensack Meridian Health includes:
To work collaboratively with the inpatient diabetes team to provide targeted education to nurses, physicians and other medical providers on the principles and practices of evidence-based inpatient diabetes management.
To serve as a main coordinator or project lead for the development and implementation of new workflows and systems that are particularly designed to optimize diabetes management.
Work with the inpatient diabetes team to develop, implement, evaluate and revise the use of protocols, policies and practice tools for diabetes management
Assist with the development and implementation of diabetes education programs
Assists physicians and other providers with quality improvement and patient-focused research-based activities.
Participates and assists with diabetes education of all staff members on medical surgical units, ER and acute care settings
Participates in multidisciplinary rounds, diabetes team meetings and other committees related to diabetes management
Actively participates in committees and attends designated meetings as a member of the diabetes team.
Other duties and/or projects as assigned.
Adheres to HMH Organizational competencies and standards of behavior.
Qualifications
Education, Knowledge, Skills and Abilities Required:
Bachelor of Science Nursing (BSN)
Graduate of an NLN/AACN accredited program in nursing
Minimum of five (5) years in diabetes patient care
Experience in staff and patient education
Experience in inpatient diabetes management
Excellent written and verbal communication skills.
Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
Education, Knowledge, Skills and Abilities Preferred:
Masters Degree
Proficiency in data management and data analytics
Maintenance of professional continuing educational requirements
Actively seeks to advance knowledge in attending appropriate seminars and programs.
Demonstrated professional investment in the commitment to inpatient diabetes education and high standards of care.
Licenses and Certifications Required:
NJ State Professional Registered Nurse License.
AHA Basic Health Care Life Support HCP Certification.
Licenses and Certifications Preferred:
Certified Diabetes Educator.
