We are seeking a Cancer Referral Coordinator who will provide administrative support within the specialized diagnostic and referral services in the Oncology Ambulatory setting. This position will primarily support the Cancer Diagnostic Center and manage incoming referrals received by external affiliations and partnerships through the Collaborative Programs. Additionally, the coordinator will help support the Molecular Tumor Board through administrative functions unique to that program. The coordinator will serve as an integral member of the clinical team and will work closely with a wide-ranging group of providers and collaborative navigators. The coordinator will ensure timely responses to referral requests, efficient medical record retrieval, and appropriate care coordination. The coordinator will assume administrative tasks, special projects, and so forth, as requested by manager and/or clinical team.
Specific Duties & Responsibilities
Serve as the primary point of contact for internal and external patient referrals to the Cancer Diagnostic Clinic and the Collaborative Programs.
Manage incoming calls and emails and triage patient referrals to clinical team.
Monitor Epic In-Basket messages and Right Fax account and ensure timely response to requests.
Inform patient of clinical documentation needed and assist patients with medical record retrieval. Obtain medical records, pathology and imaging from outside facilities.
Submit patient pathology for processing and track completion.
Coordinate additional diagnostic testing as needed, such as lab appointments, radiology imaging and biopsy. Ensure completion of necessary testing and communicate updates to clinical team.
Work closely with the new patient referral office to coordinate New Patient appointments with the appropriate disease group and provider(s).
Ensure proper follow-up with referring institutions regarding patients' plan of care and/or second opinion.
Proactively seek to improve or optimize workflow and discuss with direct management and clinical leadership, as appropriate.
Monitor and maintain referral spreadsheet, tracking all patient referrals for the Cancer Diagnostic Center and the Collaborative programs.
Monitor appointment scheduling and completion and update spreadsheet with detailed information.
Review referral patterns, volumes, and service delivery.
Build rapport with referring physician practices to facilitate timely access to JHH services.
Provide essential information, hotel accommodation locations and traveling directions to JHH facilities.
Minimum Qualifications
High School Diploma or graduation equivalent.
Five years progressively responsible related experience.
Additional education may substitute for required experience, to the extent permitted by the JHU equivalency formula.
